I. Identification of the problems.

  • Evidence of meetings with the teachers, parents(if possible), nurses, counselors and any other relevant people.

 

 

  • Needs assessment.  Address 3 domains.

 

 

  • Identify student’s problem behavior in specific, observable terms (Steps 1 & 2 worksheets in FBA; Step 1 in PBS).

 

 

II. Gather Information.

 

 

  • Student history, quality of life, interests, and schedule, learning style, environment and settings  (Steps 3 & 4 in FBA; Step 2a in PBS).

 

 

  • Determine assessment plan; explain why certain techniques were selected.  Collect behavioral baseline data on 3 to 5 data days (Steps 3, 4 & 5 in FBA; Step 2b in PBS).

 

 

III. Develop hypothesis statements. (Step 6 in FBA; Step 3 in PBS).

 

 

IV. Design a behavioral support/intervention plan. Provide rationale and material samples. (Step 7 in FBA; Step 4 in PBS).

 

 

V. Implement the plan. Measure behavior on at least 5 data days. (Steps 8 & 9 in FBA; Step 5 in PBS).

 

 

VI. Evaluate the plan. Follow-up meeting notes on  plan’s effectiveness and recommendations for future. (Step 10 in FBA; Step 6 in PBS).
